What You’ll Do
- As the Social Services Coordinator, you’ll be at the heart of resident advocacy and support. Your responsibilities will include:
- Promoting dignity, respect, and individuality for every resident
- Maintaining accurate documentation, reports, and care plans
- Facilitating resident and family care plan meetings
- Supporting residents and families in connecting with community resources
- Participating in quality assurance, improvement initiatives, and regulatory surveys
- Coordinating student interns and collaborating with interdisciplinary teams
- Assisting residents with appointments and providing off‑campus support when needed
What You Bring
- We’re looking for someone who embodies compassion, professionalism, and teamwork. Ideal candidates will have:
- Knowledge of state and federal long‑term care regulations
- Strong communication, assessment, and documentation skills
