This is a full-time, on-site role based in Trenton, NJ, for a Director of Clinical Case Management. The role involves leading and managing a team of case managers to develop and implement effective care strategies for members. Key responsibilities include overseeing case management operations, ensuring compliance with healthcare regulations, coordinating care plans, analyzing healthcare data, and working closely with internal teams and stakeholders to drive better health outcomes and cost efficiency. The Director will play a pivotal role in enhancing patient care delivery and program success.

• Proven leadership and management skills with experience leading cross-functional teams in healthcare or clinical case management.
• Expertise in care planning, case management, and coordinating multidisciplinary healthcare teams.
• Knowledge of healthcare regulations, compliance standards, and best practices in clinical settings.
• Data-driven decision-making skills, including experience with healthcare analytics and reporting tools.
• Excellent communication, problem-solving, and interpersonal skills with the ability to collaborate across teams.
• Ability to work on-site in Trenton, NJ, and travel as needed for stakeholder engagements.
• Registered Nurse (RN) or licensed clinical professional preferred; other certifications in healthcare management or case management are advantageous.
• Bachelor’s degree in Nursing, Healthcare Administration, Public Health, or related field required; Master’s degree is a plus.
