POSITION TITLE: Parent Navigator
PROGRAM: Community Support for Families
GENERAL STATEMENT OF RESPONSIBILITIES:
Community Support for Families Parent Navigator(s) provides direct support and service coordination to families in Region 5 CSF. CSF Parent Navigators utilize strength-based, family-driven, youth-guided, culturally responsive principles to develop individualized mentoring and engagement with families. The purpose of Community Support for Families is to ensure that services are delivered in a coordinated and therapeutic manner, and are consistent with the core values and guiding principles of the Strengthening Families and WRAP-CT principles. Parent Navigators work with the CSF worker to link families to formal and informal supports in the community.

EXAMPLES OF RESPONSIBILITIES:
- Accept referrals as assigned by the CSF Supervisor and share a “caseload” of approximately 12 families with CSF workers.
- Meet with families at a time and location that is most convenient to them with the CSF workers. This may include doing home visits on evenings, weekends and holidays, as needed. The CSF Parent Navigator will not do home visits alone.
- Maintain weekly in-person and/or phone contact with families served and
document all contacts.
- Using the WRAP process, assist the CSF worker in completing the Family Record, which includes the identification of Family Strengths, Vision, Needs, and Strategies in the development of a Plan of Care.
- Assist the families in obtaining needed resources, including referrals to FAVOR network of support and training.
- Assist families in setting up and convening Family Team Meetings (FTM) meetings – at least once per month – to develop, implement, and monitor progress on their Plan of Care.
- Assist families to access culturally appropriate resources to support them in crises, using their proactive and reactive safety/crisis plan.
- Assist with outreach efforts in the region to facilitate referrals through the local systems of care Collaborative, promote family involvement in the local Systems of Care Community Collaboratives and support other family initiatives.
- Maintain records on all CSF children and families. Comply with all documentation guidelines established by Wellmore.
- Maintain confidentiality in management of records and communications regarding families – including obtaining appropriate consents to release information.
Qualifications
Education and Experience:
Community Support for Families Parent Navigators should possess a college degree (AA/AS or BA/BS) in a human service field (e.g., social work, marriage and family therapy, counseling, or psychology, etc.). Persons with a degree in an unrelated field or without a college degree may be considered if they have at least 3 years’ experience working with families and knowledgeable about community resources and services preferred. CSF Worker must have the skills necessary to meet the cultural and linguistic needs of the families to be served by the program. Bi-lingual (Spanish English) may be required.
Experience with advocacy, special education services and policies, and work with children and families is preferred. Parents of their own special needs’ children are encouraged to apply.
The successful candidates must have the ability to maintain the utmost confidentiality. Must be capable of both working independently and as part of a team.
