Role Description
This is a full-time 9-5 role for a Care Coordinator, located in Huntington, NY. The Care Coordinator will be responsible for coordinating and managing care for individuals with chronic medical conditions and or, mental illness. Daily tasks include creating and implementing care plans, link and refer to community resources, and regularly meeting with clients.
Qualifications
- Care Coordination and Case Management skills
- Strong Communication and Customer Service skills
- Excellent organizational and time management skills
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field
- Experience working with individuals facing disabilities, mental illness, or addiction is a plus
