We are looking for a Community Liaison for Home Health who is energized by growth, motivated by results, and excited by the opportunity to help shape the future of a growing home health company.
This Community Liaison / Business Development professional will help expand our presence throughout York County, PA and the 5 surrounding counties. This role is ideal for someone with healthcare services sales experience - particularly in home health or hospice - who wants more direct involvement, visibility, and influence than they may find in a large corporate organization. You will work closely with ownership and executive leadership on strategy, relationship development, market growth, and outreach initiatives while helping strengthen and grow existing referral partnerships throughout the community.
Preference will be given to candidates who have already built trusted relationships with local hospitals, physicians, senior living communities, and other referral partners. Home Health experience as a PTA, COTA or LPN is bonus.
Responsibilities include:
• Building relationships with referral sources
• Managing a pipeline of target organizations and contacts
• Scheduling and completing outreach visits and meetings
• Following up consistently with referral partners
• Tracking activity and progress in a CRM or pipeline system
• Providing weekly updates to leadership
• Maintaining relationships after referrals begin
• Communicating feedback and opportunities to the internal team
