Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a Licensed Practical Nurse to become a part of our team and help provide our villagers with the highest quality of care. Current Needs

PRN / As‑Needed Coverage: Monday–Friday, either 7:00 a.m.–3:30 p.m. or 8:30 a.m.–4:30 p.m.

Minimum Availability: At least one 7.5‑hour shift per month or two to four shorter (2–4 hour) shifts per month.

Holiday Requirement: Must be available to work one holiday per year.

Job Description
As an LPN, your scope of responsibilities will include but is not limited to:
- Successfully developing and implementing a health care program to evaluate Villager’s level of independence and need for supportive services.Determining the needs of especially frail or ill Villagers and helping to establish linkage with supportive services. Leading the discharge planning process when Villagers can no longer live safely in a cottage/apartment. Facilitating communication between members of an interdisciplinary team when considering Villagers’ care plans.
- Evaluating villagers’ needs and providing care as needed to ensure the health, welfare and safety of the Villagers. Responding to emergencies and evaluating needs, providing nursing care as needed. Collaborating with physician and/or RN as needed. Assuring that Villager’s rights are protected by the Resident’s Bill of Rights. Completing required documentation and record keeping as required.
- Reviewing pre-admission and completing new admission evaluations per Village Health Services standards for admission. Collaborating with Villagers, team members and outside agencies to identify, establish and arrange for appropriate services. Conducting new Villager orientation on health and medical services as well as safety and well-being.
- Coordinating the provision of available health care services to help assure the health, welfare and safety of the Villagers. Referring Villagers to physicians or appropriate agency as medically appropriate. Planning and evaluating the Villager’s need for nursing or Village Health Navigator regarding his/her socialization.
- Must be licensed as a practical nurse in the State of Pennsylvania.
- Post licensure experience preferred but not required.
- Must demonstrate CPR competencies.
- Evidence of continuing education in line with community, state and federal regulations.
- Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.
