Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions, including vital signs and overall health status
- Support patients with memory care needs, including Alzheimer's and dementia care, ensuring safety and comfort
- Document patient information accurately while maintaining HIPAA compliance and confidentiality
- Assist with transfers using patient transfer methods such as Hoyer lifts or other mobility aids
- Provide companionship and emotional support to clients, especially those receiving hospice or end-of-life care
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred.
- Demonstrated experience working with geriatric patients, individuals with developmental disabilities, or those requiring dementia or Alzheimer's care
- Knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) or CPR certification required
- Familiarity with home health agency operations, home & community care services, and hospice care standards preferred
- Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or other patient transfer devices
- Experience working within home health regulations and maintaining detailed medical documentation is highly valued
- Strong communication skills for effective client interaction and collaboration with healthcare teams

5a-7a Saturdays
12p-1p Saturday/Sundays
6p-7p Friday/Saturday/Sunday
Pay: $22.00 - $25.00 per hour
