The Personal Care Provider delivers compassionate, person-centered services to individuals in their homes and communities who require assistance due to age, illness, disability, or other challenges.
Services may include personal care, activities of daily living (ADLs), light housekeeping, meal preparation, transportation, companionship, and respite care.
The Provider ensures all services are delivered in a respectful, safe, and professional manner consistent with Agency policies, client care plans, and industry standards.
Responsibilities/Activities:
Home Support
- Maintain a clean, safe home environment by performing light housekeeping tasks such as making beds, dusting, vacuuming, washing floors, cleaning bathrooms and kitchens, and doing laundry.
- Assist with meal planning and preparation, following special diet plans as directed by a physician or outlined in the care plan.
Complete essential shopping or errands as assigned, following Agency policy and Supervisor direction.
Companionship and Respite
- Provide companionship and emotional support through conversation, social engagement, and activities that promote mental stimulation and well-being.
Offer respite services for family caregivers in alignment with the client’s care plan.
Client Safety and Observation
- Promote client safety by supervising the home environment and reporting unsafe conditions or changes in client status to the Supervisor promptly.
Document daily activities, observations, and service hours accurately and on time.
Communication and Collaboration
- Participate as a member of the Care Team, contributing input and feedback to improve client care and program quality.
- Communicate effectively and respectfully with clients, families, supervisors, and co-workers.
Attend orientation, in-service training, and staff meetings as required.
Administrative Duties
- Follow written care plans and Agency procedures.
- Maintain accurate records and documentation.
- Monitor supplies and resources used in the provision of care.
Perform other duties as assigned by the Supervisor.
- Personal care and home management principles and techniques.
- Standards for providing high-quality, person-centered care and maintaining client satisfaction.
- English language proficiency (written and verbal).
- Basic knowledge of emergency procedures.
- General clerical skills including recordkeeping and documentation.

Qualifications/Education
- Home Care Aide (HCA) or Certified Nursing Assistant (CNA) certification preferred.
Training and certification may be available through the Agency - Valid driver’s license and reliable transportation.
- Proof of current vehicle insurance.
- CPR and First Aid certification preferred.
Training/Experience:
- Minimum one (1) year of caregiving or related experience preferred.
- On-the-job training provided for new or expanded responsibilities.
