Job Title: Licensed Practical Nurse (LPN)
Reports To: Clinical Director and Director of Professional Services
FLSA Status: Non-Exempt
Position Summary
The Licensed Practical Nurse (LPN) provides comprehensive skilled nursing services to assigned patients in their homes. Care is focused on promoting recovery, independence, and quality of life while ensuring compliance with Medicare, Medicaid, state regulations, and agency policies. The LPN collaborates with the interdisciplinary team to deliver safe, effective, and patient-centered care.
Education & Experience
- Current LPN license in the State of Oklahoma.
- Minimum of one (1) year experience as a Licensed Practical Nurse preferred.
- One year of home health nursing experience preferred.
Essential Duties & Responsibilities
Patient Care
- Assess and evaluate patients’ needs, medical necessity, and homebound status.
- Provide individualized nursing care using the nursing process, involving families as appropriate.
- Perform ongoing nursing assessments and document care accurately.
- Assist in discharge planning from admission through the course of care.
- Report significant changes in condition to the physician and follow up with patients and families.
- Maintain infection control and safety practices in the home environment.
- Make scheduled patient visits and provide care according to the plan of care.
- Participate in on-call rotation as assigned.
Documentation & Compliance
- Complete skilled nursing visit documentation within 24 hours, including wound care notes, change orders, and updated medication profiles.
- Ensure documentation reflects compliance with standards of care and updates to the patient plan of care.
- Correct documentation errors daily and ensure accuracy.
- Attend case conferences at least every 60 days and provide input into the individualized plan of care.
- Participate in CQI/quality improvement activities and compliance program initiatives.
- Submit timely and accurate records, including case conference notes, care plans, and summaries.
Teamwork & Supervision
- Communicate effectively with staff, supervisors, patients, families, and physicians.
- Collaborate with case managers and interdisciplinary team members to review patient care.
- Assist in orienting and supporting new LPNs and CHHAs.
- Provide coverage in the office or field as needed.
- Attend staff meetings, in-services, and trainings as required.

- Maintain required annual skills checklists and certifications.

- Proficiency with computers and electronic health records.
- Current CPR certification and compliance with personnel requirements.
- Strong organizational, planning, and time management skills.
- Ability to multi-task, problem-solve, and make sound clinical decisions.
Physical Requirements
- Ability to work full-time (minimum 40 hours per week) and participate in on-call rotation.
- Ability to sit, stand, walk, stoop, kneel, and bend as required.
- Ability to lift, push, or pull up to 75 pounds and transfer patients using proper body mechanics.
- Ability to write or use a computer for up to 5–6 hours per day.
- Ability to tolerate stressful environments and varied home settings.
- Ability to drive to patients’ homes within the agency’s service area; must maintain reliable transportation with valid insurance.
- May be exposed to communicable diseases and certain unpleasant patient care activities.
