A flexible schedule with hours that are consistent working virtually and in the office / field .
Our mission is to bridge the gap in healthcare systems through partnerships with other organizations such as the Meals on Wheels and Healthcare Providers.
Our primary focus is on our members. We work to ensure that they stay healthier in the comfort of their homes, avoid unnecessary hospital visits, and stay healthy in their job positions.

The Personal Health Nurse (PHN) RN is a vital part of our organization that helps change the lives of the members we serve. The PHN RN is responsible for providing clinical supports in the organization. Other responsibilities include :
- Daily calls to members to check on their well-being
- Monthly ride-along with meals delivery drivers to enroll members
- Calls to healthcare providers based on the members need
- Documentation of service calls and clients’ exceptional communications
- Facilitation of grant funded-Chronic Diseases and Diabetes Self-Management classes and Bingocize programs in the local area
- Care Planning
- Monthly members home visits
