WeCare is growing! To prepare for this growth we are seeking to identify candidates for our next orientation group to join our team of Personal Health Nurses. This position is primarily office based and requires some field work while also offering limited at-home virtual work.
Our mission is to bridge the gap in healthcare systems through partnerships with other organizations such as the Meals on Wheels and Healthcare Providers.
Our primary focus is on our members. We work to ensure that they stay healthier in the comfort of their homes, avoid unnecessary hospital visits, and stay healthy in their job positions.

The Licensed Practical Nurse (LPN-Personal) is a vital part of our organization that helps change the lives of the members we serve. The LPN (Personal) is responsible for providing clinical supports in the organization. Other responsibilities include :
- Daily calls to members to check on their well-being and provide on-line CDSME training
- Monthly ride-along with meals delivery drivers to enroll members
- Calls to healthcare providers based on the members need
- Documentation of service calls and clients’ exceptional communications
- Occasional clerical works
- Care Planning
- Monthly members home visit
