About the Role

You won’t just be completing assessments—you’ll be building care plans that truly impact lives, working alongside a collaborative team, and helping ensure every member receives the support they deserve.
What You’ll Be Doing
As part of our interdisciplinary team, you will:
Lead Member Assessments
Conduct thorough evaluations of physical, mental, and psychosocial needs for both new and existing members.
Create Meaningful Care Plans
Collaborate with nurses, care managers, and other professionals to design personalized care strategies.
Be the Communication Hub
Serve as a trusted liaison between members, families, insurance providers, and care teams.
Ensure Accuracy & Compliance
Document and report data with precision to meet Medicare, Medicaid, and regulatory standards.
Support Quality Care
Stay current with AFC guidelines and contribute to improving care quality across the organization.
Stay Connected
- Visit members and caregivers monthly until services begin
- Step in to support fellow nurses when needed
What You’ll Need
- Active Registered Nurse (RN) license in Massachusetts
- Reliable transportation for statewide travel
- 3 years of MDS experience
- Experience with EMR systems
- Strong critical thinking, organization, and communication skills
- Knowledge of HIPAA and healthcare regulations
- Relevant certifications
