Duties
- Conduct comprehensive assessments of patients to complete MDS documentation accurately.
- Collaborate with healthcare professionals to gather necessary medical records and patient information.
- Ensure compliance with all regulatory requirements related to MDS submissions.
- Maintain up-to-date knowledge of changes in regulations affecting MDS processes.
- Facilitate interdisciplinary team meetings to discuss patient care plans and outcomes.
- Monitor and analyze data related to patient care quality metrics.
- Provide training and support to staff on MDS-related processes and documentation standards.
- Utilize electronic health record systems effectively to manage patient information.
- Proven experience in a medical office or healthcare setting, Skilled Nursing Facility a must!
- RN or LPN License active in the state of Massachusetts
- Strong knowledge of medical documentation practices and patient care protocols.
- Familiarity with electronic health record systems such as Point Click Care is highly desirable.
- Excellent organizational skills and attention to detail for accurate data entry and record keeping.
- Ability to work collaboratively in a team-oriented environment while managing multiple priorities.
- Strong communication skills, both verbal and written, for effective interaction with patients and staff.
- Background in emergency medicine is a plus, enhancing the understanding of urgent patient needs.
