Registered Nurse (RN) – AFC/GAFC Home Care – Cape Cod
We are seeking a compassionate and organized Registered Nurse (RN) to join our growing AFC/GAFC team serving the Cape Cod area.
This role is ideal for an RN looking for flexible scheduling, meaningful long-term patient relationships, and a lower-stress alternative to traditional facility or hospital nursing.
Responsibilities
• Complete initial nursing assessments and reassessments for AFC/GAFC participants
• Develop and maintain care plans
• Conduct routine supervisory visits and documentation
• Coordinate with caregivers, families, physicians, and community providers
• Monitor participant health status and identify changes or concerns
• Ensure compliance with MassHealth and program requirements
• Support caregivers with education and guidance
Qualifications
• Active Massachusetts RN license
• Valid driver’s license and reliable transportation
• Strong communication and organizational skills
• Ability to work independently and manage scheduling efficiently
• Home care, case management, AFC, or community nursing experience preferred
