General Description of Job
General activities of the Employee/Paraprofessional are aimed at encouraging the Participant to become as self-directing as possible within medically determined limitations. Also, to help the Participant maintain good standards of hygiene; and to stimulate mental alertness through conversation and participation in individualized appropriate activities.
Responsibilities and Duties
Is responsible for providing residential habilitation services in a safe environment. Duties include the performance of simple procedures as an extension of therapy services, personal care, ambulation and exercise, passive range of motion activities, household services essential to residential habilitation, assistance with medications that are ordinarily self administered, teaching habilitation skills, recording changes in the Participant’s conditions and needs and progress towards goals as per individual support and habilitation plan and completing appropriate records.
1. Providing personal care, habilitative and supportive services as per Participants Individual Support Plan.
2. Maintain adequate and complete training, education, and experience to effectively and properly perform all duties listed herein, and those otherwise assigned.
3. Maintain all applicable legally – and regulatory – required qualifications and/or certifications.
4. Complete and maintain current CPR certification and First Aid training.
5. Maintain current and valid automobile liability insurance covering each and every vehicle used by the employee in connection with, or relating to, the performance of the duties listed herein, and those otherwise assigned.
6. Complete W.D.B., Inc.’s orientation and training programs as assigned.
7. Participate, at employee’s expense, in a minimum of 8 hours of in-service training each year.
8. Assisting with activities of daily living (ADL), personal hygiene, eating and meal preparation, dressing, toileting skills as needed, exercises and ambulation. All daily activities.
9. Maintaining a safe environment.
10. Maintaining current and accurate clinical records.
11. Communicating to the Program Coordinator (QIDP) changes in the status of the Participant.
12. Providing household duties to facilitate the Participant’s self-care at home, and giving the Participant opportunities to be as independent as possible.
13. Participating in Participant Care conferences and in-services.
14. Assisting Participants in establishing a daily schedule for household duties and activities of daily living.
15. Instructing Participants in learning and re-learning household duties and activities of daily living.
16. Assisting with laundry, bed making and changing of bed.
17. Assisting in promoting Participant’s alertness and reality orientation through conversation, companionship and other activities.
18. Assisting Participants with prescribed activity and exercises in accordance with instructions of appropriate professional personnel.
19. Employee/Paraprofessional will model appropriate Socialization Skills to the Participant.
20. Employee/Paraprofessional will assist Participant to and from activities in the community and will assist with supervising Participant during community activity as needed
21. Employee/Paraprofessional will teach Participant money management skills as indicated in the Participants Individual Support Plan.
22. Employee/ Paraprofessional will teach Participant friendship/relationship skills as indicated in the Participants Individual Support Plan.
23. Employee/ Paraprofessional will facilitate safety skills.
24. Employee/ Paraprofessional will assist Participant with Behavioral Management as per Individual Support Plan and Behavioral Plan as needed.
25. Employee/ Paraprofessional will be respectful of the Participant and act as an advocate for the Participant.
26. Employee/ Paraprofessional will assist Participant with developing communication skills.
27. Employee/ Paraprofessional will assist Participant with dealing with losses and grieving process as needed.
28. Employee/ Paraprofessional will assist Participant with hobby development as per Individual Support Plan.
29. Employee/ Paraprofessional will do habilitation training as per Participants Individual Support Plan.
30. Employee/ Paraprofessional will accurately bill for services rendered to Molina on the DSL (Daily Service Log).
31. Employee/ Paraprofessional is responsible for Participant on a twenty-four hour basis.
32. Employee/ Paraprofessional will assist with accessing and training of Alternative Care Employee/ Paraprofessional as needed and will communicate to Program Coordinator when Alternative Care Employee/ Paraprofessional will be utilized.
33. Employee/ Paraprofessional will assist with training of new employees.
34. Employee/ Paraprofessional will not ask Participant’s for money to reimburse for expenses. There will be no mingling of funds between employee/staff at any time and for any reason.
35. Employee/ Paraprofessional will not sell, buy or trade merchandise with a Participant.
36. Employee/ Paraprofessional will not give their personal phone number to a Participant.
37. Any other duty deemed necessary by the manager or administrator
Requirements:
JOB SPECIFICATIONS
Age
Must be eighteen years or older.
Education
High School graduate or equivalent preferred. Be able to read and write legibly, to understand and carry out directions and instructions, keep simple records, and function with minimal supervision.
Completion of CPR and First Aid
Course before official start date working with participants.
Other
Valid Driver’s License
Criminal background check satisfactorily completed online and passed within 60 days of hire.
