What are the responsibilities and job description for the On-Call Hospice RN, Per Diem position at Waveny Brand?
Implements, coordinates and supervises all aspects of delivery of patient care for hospice program:
- Triages incoming calls for referral or care and implements appropriate plan (i.e. home visit, professional consultation, interagency referral, etc.)
- Complies with Agency documentation standards.
- Provides consultation and visits when appropriate for pain management, symptom management, comfort care, HHA orientations/supervisions, Death Pronouncement and bereavement care
- Maintains ongoing communication with IDG staff.
- Calls in report before 8:30 a.m. to all appropriate parties on patient issues/contacts during evening/night.
- Seeks consultation with other agency staff as indicated.
- Participates in Hospice IDG Team and other meetings, as requested.
Educational
- Certified in CPR
- Attends yearly OSHA in-service and completes annual skills validation
- Identifies knowledge gaps and remedies independently
- Attends minimum 12 hours of in-services per year, including 6 hours of hospice in-service.
Minimum qualifications

- Current license to practice as RN in Connecticut.
- BSN degree preferred.
- Valid driver’s license
- Minimum three years clinical experience in the past five years, at least one of which must be palliative care, end of life care, long-term care, or hospice care.
- An understanding of and commitment to hospice principles and philosophy.
- Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.
