Key Responsibilities
Complies with facility documentation standards. Maintains open communications with all members of the interdisciplinary healthcare team. Ensures Interim baseline care plans are within 24 hours of admissions. Rounds with consulting physicians (cardiac, wound care, etc.).
Serves as a clinical coordinator, providing clinical guidance and hands on care as indicated, according to best practice guidelines and facility standards of care.
Functions as clinical team leader. Directs, instructs, supports, and supervises ancillary personnel to assure that the needs of the primary assignment and patient’s care plan are met.
Assures and maintains a safe and infection free environment.
Establishes/expands rehabilitative nursing skills on an ongoing basis. Maintains proficiency in short term rehab care, terminal care, and mandatory skills.
Supports, promotes, and projects guest relations and positive images to all visitors, patients, and other Waveny LifeCare Network personnel.
Physical Requirements/Work Experience:
- Standing/waking during periods of the day
- Sitting and typing more than half the day
- Moving/lifting objects 5-300 lbs.
- Will be exposed to rehab patients with medical/surgical needs and equipment
- Will be exposed to terminal and long term patients
- May be exposed to patients with psychiatric illness or dementia
- May be exposed to needles, sharp objects, and bodily fluids
- May be exposed to infectious diseases
- May be exposed to moderate stress and injury risk from patient contact.
