Position: Nurse Practitioner (Full Time)

Position Summary
We are seeking a dedicated, patient-centered Nurse Practitioner to join our clinical team. This role will serve both residents and staff, providing primary care, acute and chronic disease management, and collaborating with interdisciplinary teams within our senior living environment. The NP will be a key contributor to enhancing access to care, promoting wellness, and coordinating care across our on-site health assets (e.g. therapy clinic, home health, memory care, skilled nursing) and external providers.
Key Responsibilities
- Provide primary and acute care services to residents and staff as needed (history/physicals, evaluation and management, diagnosing, prescribing, referrals)
- Manage chronic disease care (e.g., hypertension, diabetes, COPD, heart failure)
- Perform wellness visits, health assessments, and preventive care
- Collaborate with physicians, nursing teams, therapy, pharmacy, and home health to coordinate care plans
- Make on-site rounds (assisted living, memory care, skilled nursing) as needed
- Order, interpret, and follow up on diagnostic tests (labs, imaging)
- Participate in quality assurance, performance improvement, and risk management initiatives
- Engage in resident/family education and health promotion programs
Qualifications / Requirements
- Master’s or Doctoral degree in Nursing (NP program)
- Active Florida ARNP license and appropriate NP specialty certification
- DEA registration (or ability to obtain)
- Minimum of 2–3 years of NP experience preferred, ideally in geriatrics, primary care, or senior living / long-term care
- Strong clinical judgment, ability to work independently
- Excellent communication, interpersonal, and collaboration skills
- Familiarity with electronic health record (EHR) systems
- Basic life support (BLS) / advanced certifications as required
