Join our dynamic healthcare team as an Autonomous Advance Registered Nurse Practitioner (ARNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse clinical settings. Your expertise will help elevate the quality of care provided, making a meaningful difference in patients’ lives every day. This position will involve rounding at 2 ALF locations. Majority of PCP patients, some minor wound care.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform accurate diagnoses.
- Prescribe medications and treatment plans in compliance with state and federal regulations, including DEA regulations for controlled substances.
- Perform advanced procedures such as suturing, IV insertion, catheterization, spinal taps, and dermal filler injections to support diverse patient needs.
- Manage complex cases involving acute pain management, critical care scenarios, telehealth consultations, and chronic disease management across specialties like gastroenterology, geriatrics, behavioral health, and hospice & palliative medicine.
- Collaborate effectively with healthcare professionals—including physicians, specialists in infectious disease care, cardiology, ophthalmology, and others—to develop comprehensive care plans.
- Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate medical documentation, specimen processing, and medical records management.
- Engage in patient education and health coaching to promote wellness initiatives and adherence to treatment protocols.
Requirements
- Valid licensure as an Advanced Practice Registered Nurse (APRN) with authorization for autonomous practice in the relevant state or jurisdiction.
- Proven experience working with diverse populations including pediatric (experience working with children), adult, geriatric (geriatrics), and special needs patients such as those with developmental disabilities or dementia.
- Demonstrated proficiency in EMR/EHR systems like Epic or Athenahealth; experience with telehealth platforms is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology (including CPT coding), and diagnostic evaluation techniques such as sonography or laboratory testing.
- Hands-on experience performing advanced procedures including suturing, sterile processing techniques, phlebotomy, venipuncture, catheter care, tube feeding management, and airway management.
- Critical care experience from ICU or Level I/II trauma centers; hospital medicine or emergency room experience is preferred.
- Familiarity with healthcare laws including HIPAA compliance and Workers’ Compensation law; knowledge of ICD-10/ICD-9 coding is essential.
- Excellent client communication skills coupled with the ability to work independently while collaborating within multidisciplinary teams across settings like nursing homes, assisted living facilities, outpatient clinics, or post-acute care environments.
