SUMMARY

This Clinical Pharmacist position is responsible for being a hospital staff pharmacist that delivers high-quality, patient-centered pharmaceutical care across the inpatient setting, outpatient treatment and infusion center, and emergency department as needed. The Clinical Pharmacist is responsible for overseeing compounding or compounding medications directly when the need arises – including hazardous drugs such as chemotherapy – providing expert clinical input during patient rounds, and ensuring safe and effective medication use throughout the hospital stay. Additionally, this position serves as a vital clinical resource for providers, contributing to optimal patient outcomes in both acute and ambulatory care environments.

DUTIES/RESPONSIBILITIES (include but are not limited to the following)

  • Provide pharmaceutical services to the Inpatient department (22 bed) and to the Outpatient Treatment and Infusion Center.
  • Evaluation of medication therapy, clinical assessment of patients, medication reconciliation, medication management, pharmacokinetic consults, resolution of emergent lab values, management of adverse drug reactions, therapeutic drug monitoring, and discharge planning.
  • Dispense and compound sterile medications, including but not limited to chemotherapy, and/or supervises this process.
  • Provide accurate, efficient and appropriate pharmaceutical distribution services to patients by properly interpreting providers drug orders and dispensing the appropriate medications.
  • Communicate with after-hours pharmacy service staff to ensure transition of care.
Qualifications:

EDUCATION AND/OR EXPERIENCE

  • Degree in Pharmacy (PharmD) from an accredited college and State of Iowa licensure to practice pharmacy and to dispense controlled substances.
  • An ASHP-accredited residency in clinical/hospital pharmacy, or equivalent experience, is highly desired.
  • BLS certification or obtain within 30 days of hire
  • Experience in chemotherapy preparation is preferred; however, training wil be provided for candidates demonstrating strong clinical aptitude and a commitment to learning.
