CASE MANAGER
PURPOSE: The Case Manager of the Washington County Homeless Project is responsible for supporting the organization’s mission. The Case Manager will provide support to the Executive Director in providing the day-to-day operations of the organization.
REPORTS TO: Executive Director
ESTIMATED HOURS PER WEEK: 30
RESPONSIBILITIES:
1. Conduct assessments to evaluate clients’ needs.
2. Work with clients to develop appropriate strategies and plans for addressing their issues.
3. Refer clients to community resources and services.
4. Assist clients with paperwork to receive needed services.
5. Assist clients with obtaining medical and/or mental health appointments.
6. Maintain strict confidentiality and a professional demeanor at all times regarding all matters pertaining to WCHP and its constituents.
7. Assist clients as needed by arranging for transportation to scheduled appointments.
8. Assist clients to apply for birth certificates, IDs, housing and employment applications, social security benefits and other resources as needed.
9. Maintain accurate client records, documenting appointment contacts, consents, case management notes and include all documentation needed for continuity of care.
10. Must have organizational skills and work independently with minimal supervision.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required.
· High school diploma or equivalent required.
· Prefer Case Management experience.
· Prefer social services experience.
Pay: $18.00 - $20.00 per hour
Work Location: In person
