Warren County Community Services, Inc. (WCCS) is a progressive Community Action Agency that is dedicated to creating a safe, inclusive, and caring environment for seniors. The Care Manager (CM) will assess Warren County, OH residents, age 60 and over, for eligibility in the Elderly Services Program (ESP). The Elderly Services Program helps older adults remain safe and independent in their homes. This position is a combination of work-from-home and work from the field, conducting home visits with clients located in Warren County, OH and in office time as needed.
The CM will adhere to all policies and procedures of Warren County Community Services, Inc., and Council of Aging of Southwestern Ohio. This includes continuous assessment of program eligibility, developing and monitoring service plans to meet clients’ needs, reviewing income and medical expense information, and monitoring health and safety concerns. The CM is responsible for ensuring ESP is the payor of last resort and will maintain up-to-date knowledge of community resources and other programs, including waiver programs administered by the Council on Aging of Southwestern Ohio. All duties should be performed in a fiscally responsible manner, in line with program guidelines, and with a focus on improving the quality of life for clients.
Primary Duties/Responsibilities:
- Assess client eligibility for the ESP program with accuracy and efficiency.
- Coordinate and develop client Service Plans tailored to individual needs.
- Conduct detailed assessments of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- Evaluate financial responsibility, including possible co-pay, and ensure appropriate documentation.
- Complete all required interventions in a timely and accurate manner, following contract requirements.
- Maintain thorough documentation of all actions and case notes, adhering to established documentation standards.
- Assess waiver program eligibility and refer clients to the Council on Aging as needed.
- Follow departmental standards regarding follow-up timeliness, incident reporting, mandatory reporting to Adult Protective Services (APS), and management of high-risk incidents.
- Educate clients and caregivers on their rights, responsibilities, grievance procedures, confidentiality, Health Insurance Portability and Accountability Act (HIPAA), Payor of Last Resort protocols, and the release of information process.
- Provide referrals to clients and caregivers to appropriate community resources, ensuring they receive necessary support.
- Attend staff meetings, mandatory training events, and relevant professional development sessions.
- Maintain required licensure, certifications, and training as applicable.
- Perform other duties as assigned by management.

- Participate in outreach activities within the community, as assigned.

Required Skills/Abilities:

- Must have a valid Ohio Driver’s license, proof of vehicle insurance, reliable transportation, a clear background check, and a negative drug screen.
- Proficient in Microsoft Office Suite and other software tools for documentation and communication.
- Requires working knowledge of smartphones, including using apps or tools for communication and case management.
Education and Experience:
- Required: Associate’s degree in a human services field, with at least two years of experience in home health care, medical social work, geriatrics, care/case management, or an equivalent substitute OR Bachelor’s degree in Nursing, Social Work, Gerontology, Psychology, or a related field, OR RN Licensure with 1 year of experience in home health care, medical social work, geriatrics, care/case management, or an equivalent substitute.
- Preferred: Experience working with individuals aged 60 and over, particularly in a community-based care setting.
Physical Requirements:
- Ability to sit at a desk and work on a computer for extended periods.
- Must be able to ambulate in various housing environments, including using steps or climbing multiple levels.
- Ability to work in homes that may present environmental challenges, such as hoarding, unsanitary conditions, or pest control issues.
- Must be able to lift up to 15 pounds occasionally and manage any physical demands associated with home visits.
