What are the responsibilities and job description for the Birth to Three Service Coordinator (HHS) position at Walworth County, WI?
This position is responsible for providing assessment, planning and on-going case management services to children with developmental delays, disabilities, or other atypical developments as mandated by the Federal Birth to Three program. This position is also responsible for organizing and participating in each child’s evaluation, screening, and development of the Individualized Family Service Plan, lead coordination of care, and will assist families in accessing community resources and will teach them how to be an advocate for their child. This position is governed by DHS 90.

Evaluate and assess consumers and families with children who have difficulties in psychosocial and/or developmental delays (including providing advocacy, intervention planning, psychosocial evaluating and counseling of clients and families when needed) through face to face contacts with the consumer and care giver in the home, community, and in the child’s natural environment.
Consult, coach and collaborate with parents, team members, daycare providers, and a variety of different Human Service professionals for each assigned child to ensure continuity of care and progress towards identified care plan goals.
Facilitate access to services. Coordinate care and monitor services for family on a timely basis. Determine participant eligibility and terminate consumer enrollments and services as appropriate based on program requirements.
Develop individualized family service plans and ensure the plan is carried out through direct service or coordinate of care with other providers.
Work as a team member toward the development, implementation, ongoing assessment and provision of Family-focused services to children and families in the Birth to 3 program.
Ensure program compliance with all program standards through comprehensive documentation, as required by Walworth County Department of Health and Human Services, State and Federal law.
Facilitate transition plans by providing referrals to appropriate educational, community and support services.
Act as a case manager to monitor and direct coordination of services as appropriate in accordance with Medicaid and the specific service program. Meet with consumers regularly to ensure consumer progress and facilitate transition between HHS programs and those facilitated by community partners.
Lead discussion for team meetings on consumer IFSP progress and coordinate referrals for additional services in coordination with all other service providers.
Maintain supervisory contact for case direction for all assigned consumers.
Deliver targeted, culturally appropriate information and interventions to help individuals and groups understand health promotion, policies, regulations and local code as it pertains to healthy growth and development.
Serve as a resource and advocate for health and health literacy in the county and promote healthcare equity, quality and accessibility for all including vulnerable populations, focusing on the importance of healthy growth and development.
Demonstrate a commitment to county safety and risk management efforts.

Education And/or Experience
Bachelor's degree in social work from an accredited program, early childhood education or a related field with a minimum of six months of experience working with infant and toddlers and their families. Internships may qualify. Bilingual in Spanish preferred.
Certificates, Licenses, Registrations
Valid driver's license.
Proof of minimum liability auto insurance coverage.
Access to reliable transportation to all assigned work locations daily.
Knowledge, Skills And Abilities
Knowledge of development of young children including physical and emotional development preferred.
Knowledge Of Medical Terminology And Disabilities Preferred.
Knowledge in community resources, especially for children with disabilities desired.
Knowledge of Birth to 3 highly desirable and preferred.
Knowledge of early child development required.
Knowledge of Trauma Informed Care Principles is essential.
