Walker Healthforce is seeking a Bilingual RN Case Manager with minimum 2 years of clinical experience to support a Health Insurance company based out of Michigan. This is a 12-month contract opportunity.
HOURS/SCHEDULE: Three part-time RNs:
- One available Monday, Tuesday, and Wednesday
- One available Wednesday, Thursday, and Friday
- One available Rotating schedule: Week 1: Saturday & Sunday Week 2: Monday & Tuesday
CORE REQUIREMENTS:
- Bilingual
- Critical thinking skills
- Triage skills
- Advanced Assessment skills
- Positive attitude
- Side by side experience with patients (bedside, ER experience, home health, med surg experience)
- Great attendance
- Time & caseload management
- Current unrestricted RN license in Michigan.
- Minimum 2 years of clinical nursing experience, with a background in case management preferred.
- Certification in Case Management (CCM) preferred.
- Proficiency in using telehealth platforms and electronic health records (EHR) systems.
- Excellent communication skills, both verbal and written.
- Strong critical thinking and problem-solving abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic approach to patient care.
- Flexibility to adapt to changing healthcare environments and patient needs.
- Commitment to maintaining patient confidentiality and privacy.
- Bilingual Russian/Spanish/Arabic - Preferred
JOB RESPONSIBILITIES:
- Conduct initial patient assessments and develop individualized care plans based on clinical findings and patient needs.
- Comfortable managing a high-volume caseload of ~200 Medicare members while meeting quality and performance standards
- Utilize telehealth platforms to monitor patient progress, provide health education, and coordinate necessary interventions.
- Collaborate with healthcare providers, including physicians, specialists, and allied health professionals, to ensure comprehensive care delivery.
- Assess and address patient concerns, escalating issues as needed to ensure timely resolution.
- Educate patients and caregivers on health management strategies, medications, and self-care techniques.
- Document all patient interactions, assessments, and care plans accurately and timely within electronic health records (EHR) systems.
- Coordinate referrals to community resources and support services as necessary.
- Stay informed about current healthcare trends, telehealth regulations, and best practices in case management.
- Comfortable on production, meeting production case load targets and exceeding quality expectations
