What are the responsibilities and job description for the Fellow Physician Waco Family Medicine HPM Program (3528) position at WACO FAMILY MEDICINE?
PRIMARY DUTIES:
- Provide supervised hospice and palliative medicine care as an integrated member of the hospice interdisciplinary team across inpatient, outpatient, and home- based hospice settings.
- Perform comprehensive symptom assessment and management for patients with serious and life-limiting illness, including pain, dyspnea, nausea, anxiety, depression, and delirium.
- Participate actively in hospice operations, including longitudinal patient assignments, interdisciplinary team (IDT) meetings, and coordination of care across disciplines.
- Conduct goals-of-care discussions, advance care planning, and end-of-life conversations with patients and families.
- Complete required hospice clinical and regulatory documentation, including EHR documentation and death certificates, under supervision.
- Participate in joint home visits with hospice physicians and members of the interdisciplinary team (nursing, social work, chaplaincy, etc.).
- Serve in a consultative role to support primary teams with complex medical decision-making, prognostication, and ethical considerations.
- Engage in structured teaching activities, case presentations, and scholarly or quality improvement projects related to hospice and palliative medicine.

REPORTS TO: WFM – Hospice & Palliative Medicine Program Director, Attending Physicians
EDUCATION & EXPERIENCE
- M.D. or D.O. degree from accredited school
- Completion of a residency in Family Medicine, Internal Medicine, Pediatrics, Emergency Medicine, or Physical Medicine & Rehabilitation
- Board eligible or certified in primary specialty
- Eligible for medical licensure
