Job Title: Clinical Pharmacist
Location: Remote (San Antonio, TX)
Contract duration: 6 months (Contract to Hire)
Schedule: 8am to 5pm (M-F)
Role Responsibilities:
- The client's Clinical Pharmacist is responsible for advancing Stars Part D performance and medication utilization strategies that enhance quality, reduce care complexity, and deliver value for physician clients. This role collaborates with client's leadership, physician clients, and other clinical stakeholders to improve quality, safety, and financial performance through data-driven interventions.
- Provide education to physicians, advanced practice providers, and clinical staff on value-based care initiatives.
- Analyze, share, and support performance data related to Stars Part D medication utilization.
- Support quality improvement initiatives, including medication adherence programs and evidence-based prescribing practices.
This role supports patients with multiple chronic conditions through comprehensive medication reviews and direct collaboration with client physicians to address drug therapy issues. It also contributes to the client's Managed Care Pharmacy PGY-1 residency program and serves as a clinical preceptor for pharmacy residents and/or students. This role does not include prescription dispensing responsibilities.
Job Requirements:
Education:
- Bachelor’s degree in pharmacy required; PharmD strongly preferred.
- Licensure: Active Texas pharmacist license in good standing required.
- Experience serving patient populations within the Texas market.
- Three or more years of managed care and/or ambulatory care experience.
- Demonstrated strong communication skills across diverse audiences.
- Demonstrated ability to work independently, solve problems effectively, and make timely decisions.
- Demonstrated comfort working in dynamic and evolving environments.
- Demonstrated ability to build and maintain strong internal and external relationships.
- Strong organizational, prioritization, and execution skills.
- Proficiency in Microsoft Word, Excel, and PowerPoint.
Preferred Qualifications
- Working knowledge of CMS Medicare structure and related regulations.
- Working knowledge of CMS quality improvement programs.
