Job Title – Case Manager - RN
Duration – 12 months Contract
Location – Remote
Pay – $40.00/hr on W2
Engagement Description –
The Case Manager RN leads the coordination of a multidisciplinary team to deliver a holistic, person-centered care management program to a diverse health plan population. This position is fully remote and supports members using a variety of communication channels including phone, email, text, and an online platform. The Case Manager RN uses the case management process to assess, plan, implement, monitor, and evaluate interventions across the care continuum, coordinating with providers, caregivers, and community resources.
Top 3 Required Skills/Experience –
• Minimum 3 years clinical nursing experience in clinical, acute/post-acute care, or community setting
• 1 year case management experience in a managed care setting strongly preferred
• Excellent communication and coordination skills, comfortable working in a fully remote, multi-system environment
Required Qualifications –
• Nursing Diploma or Associate’s degree in Nursing (BSN preferred)
• Current, active, and unrestricted Compact RN license
• Certification in Case Management (CCM) or willingness to obtain within 18 months of hire
• Intermediate proficiency in Microsoft Office (Excel, Outlook, Teams, Word, OneNote)
• Typing and computer skills test required – include score on resume
• Internet speed test and service provider must be listed on resume
Salary : $40
