Job Title: Behavioral Health Care Coordinator
Location: Remote (Travel may be required)
Contract Duration : 3 Months
Work Hours: Monday to Friday | 8:00 AM – 5:00 PM
Key Responsibilities
- :Conduct health assessments and develop individualized care plan
- sCoordinate healthcare services, transitions of care, and community resource referral
- sEducate members on disease management, wellness programs, benefits, and available resource
- sMonitor member progress and update care plans as neede
- dCollaborate with providers, caregivers, and interdisciplinary team
- sAddress member inquiries and resolve care-related issue
- sMaintain accurate documentation and meet productivity standard
- sSupport audit readiness, quality initiatives, and special project
- sEnsure compliance with HIPAA, NCQA/URAC standards, and state regulation

Qualification
- s:Current, unrestricted RN, LPC, LMSW, LCPC, LCSW, LMFT, LPCC, LISW, LMHC, or equivalent license in the state of operation
- s.Minimum 2 years of direct clinical experien
- ce3 years of managed care, wellness, case management, or care coordination experience preferr
- edStrong care planning, health education, and medical management knowled
- geExcellent communication, customer service, and relationship-building skil
- lsProficiency with Microsoft Office, databases, and web-based applicati
- onAbility to travel within assigned territory as need
