Key Responsibilities
- Provide comprehensive primary care to adult patients
- Conduct patient assessments, establish diagnoses, and develop treatment plans
- Manage acute and chronic medical conditions
- Prescribe medications and therapies as appropriate
- Educate patients on preventive care, wellness, and disease management
- Collaborate with physicians and multidisciplinary care teams
- Participate in quality improvement initiatives and performance metrics
- Support implementation of new clinical programs and care models
- Contribute to a positive, team-oriented clinical environment
- Maintain accurate and timely documentation in the EHR
- Ensure compliance with privacy regulations and clinical protocols
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Active and unrestricted state license
- Current BLS certification
- Active DEA registration
- Minimum 1 year of primary care experience
- Ability to provide professional references
