We are seeking a compassionate and dedicated Case Manager to join our Homeless Veterans Reintegration Program (HVRP) team in Cleveland, Ohio. This role focuses on providing comprehensive case management services to homeless veterans, helping them overcome barriers to employment, housing, and self-sufficiency.
The ideal candidate will have strong interpersonal skills, a deep understanding of veterans’ needs, and experience in social services and community resource coordination.
Responsibilities:
- Assess Needs & Develop Service Plans
- Conduct comprehensive assessments to identify veterans’ strengths, challenges, and goals.
- Develop and implement individualized service plans tailored to each veteran’s employment, housing, and supportive service needs.
- Employment & Financial Stability Support
- Connect veterans to job training, employment opportunities, and supportive services.
- Work closely with the Job Readiness Specialist to ensure veterans receive job search assistance, interview preparation, and resume support.
- Conduct financial assessments and provide budgeting guidance to promote financial stability.
- Housing Assistance & Stability
- Assist veterans in securing safe and stable housing, including emergency shelters, transitional housing, and permanent housing programs.
- Work with housing agencies, landlords, and community partners to coordinate placement and rental assistance.
- Support veterans in accessing housing subsidies, utility assistance, and other resources to maintain stability.
- Resource Coordination & Supportive Services
- Refer and connect veterans to healthcare, mental health, and substance abuse services.
- Assist veterans in navigating VA benefits, Social Security, and other public assistance programs.
- Provide access to transportation assistance, work attire, and tools necessary for employment success.
- Case Management & Advocacy
- Maintain accurate case files, documentation, and reporting in compliance with program requirements.
- Build and maintain relationships with service providers, employers, and community organizations to enhance veteran support services.
- Advocate for veterans’ rights and needs, ensuring they receive timely and appropriate assistance.

Applicants must have a Bachelor’s degree in the area of human and/or social services or the behavioral sciences. Licensed Social Worker designation is highly preferred. Education and/or verifiable work experience with at-risk populations is required (homeless, veterans, behavioral health, drug/alcohol addiction, employment, youth, residential).
Salary : $40,000 - $42,000
