Summary / Objective

The Outpatient Pastoral Care Coordinator collaborates with the clinical team to provide spiritual support and skills training to individuals receiving outpatient services. This includes providing coverage for crisis calls, as well as providing spiritual support through one-on-one engagement and group facilitation. This role also engages with ministry volunteers and stewards relationships with churches and faith-based organizations in the community to support client recovery and spiritual growth.

 

Essential Functions

  • Provides individual spiritual support opportunities, group facilitation, and skills training to patients.
  • Participate as an integral member of the multidisiplinary treatment team.
  • Attend team meetings, trainings, and community partner meetings as assigned.
  • Facilitate life/medical safety, behavioral control, and crisis prevention activities within the scope of practice and contact supervisors, primary clinicians, and appropriate emergency services when needed.
  • Collaborate with clinical staff as needed to intervene in situations requiring professional support, increased intervention, alternative placement, or level of care.
  • Transport clients to access services and appointments as authorized by program needs and treatment plan.
  • May administer Drug and Alcohol screens/tests as needed and authorized by clinical staff or primary therapist.
  • Utilize assessments with clients as outlined in the program, such as the DLA-20, MIS, ORS/SRS, PHQ-9, GAD 7, LOCUS, SOWS, or CIWA.
  • Maintain case records and other documentation as outlined in agency policies.
  • Maintain productivity expectations as determined by leadership.
  • Provide referrals to churches, ministers, and spiritual mentors for clients seeking spiritual guidance or Bible-based support.
  • Build and maintain strong relationships with churches, ministers, and spiritual organizations in the community.
  • Conduct ministry presentations for community partners, churches, donors, and volunteers as needed.
  • Coordinate volunteer participation in ministry activities that support clients.
  • Monitor client participation in pastoral care groups and activities.
  • Assist staff with patient discharge planning by connecting clients to churches and spiritual resources in their home communities to support continued recovery and wellness.
  • Support all local ministry services and activities
  • Other duties as assigned

  • Work with potentially difficult people in crises.
  • Maintain the confidentiality of information to include protected health information.
  • Work with a team in developing and updating the curriculum for CEP groups.

Supervisory Responsibility

This position has no direct supervisory responsibilities but does serve as a coach and mentor for other positions in the organization.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as laptop computers, photocopiers, and smartphones.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision and the ability to adjust focus. This would require the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are generally Monday through Friday, 8:00 a.m. to 5 p.m., with some occasional evening and weekend work required.

Travel

Some travel is expected for this position.

Required Education, Experience or Eligibility Qualifications

  • Background in Christian ministry/Ordination with four years' experience in the field

  -Or-

A degree in Christian counseling/Biblical studies with 1-year experience in an equivalent field.            

  • Lived experience allowing the employee to complete Peer Certification training and provide Peer Support group/individual care. 
  • Must have a current and valid driver's license with no restrictions to become fully insurable.
  • Will become commissioned as a Volunteers of America minister within the first year of employment.

 

Preferred Education and Experience

  • Bachelor's degree in Christian counseling/biblical studies from an accredited Theological Seminary with one or more years of experience in the field.
