What are the responsibilities and job description for the Community Health Liaison position at Volunteer Behavioral Health Care System?
Community Health Liaisons were designed to address a critical service gap. They are a comprehensive resource, serving as crisis assessors, care managers, educators, and community outreach specialists. The goal of BHLs is to enhance the efficiency of behavioral health systems and maximize opportunities to identify and connect individuals with the appropriate level of care. Their primary objective is to support individuals who are involved with services from other community partners, and upon discharge, connect them with necessary treatment to continue on their path to recovery. By doing so, BHLs play a vital role in the "no wrong door" model, as they are stationed in locations where individuals with behavioral health needs may not be seeking treatment, including underserved areas, and places people frequently congregate, but may not know how to access care.
Job Description:
1. Establish and maintain productive relationships with community partners. Promote VBHCS and services offered, showcasing our commitment to enhancing the overall well-being of our clients.
2. Evaluate clients to determine the level of behavioral health care they require. Provide efficient linkage to appropriate resources, ensuring each individual receives the necessary assistance.
3. Conduct crisis assessments and refer clients to the appropriate level of care for behavioral health treatment.
4. Assist with the application process for TN Safety Net and/or TennCare as needed to encourage follow through with behavioral health treatment and care.
5. Provide education and training to individuals and agencies concerning behavioral health to increase behavioral health literacy and to reduce stigma.
6. Participate in staff meetings, training, and supervisory sessions as required
7. Identify resources to assist individuals, agencies, families, and other social supports.
8. Advocate/ communicate patient needs to community partners.
9. Identify and facilitate access to other community supports.
10. Provide information and education on accessing services.
11. Provide customer service by educating clients and agencies on open access/crisis services.
12. Collect and document data required by grant funder(s) and for billing purposes, according to quality management standards and procedures.
13. Other duties as assigned.
EDUCATION & EXPERIENCE:
Master’s preferred, must have a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences and at least two (2) years of paid work experience in the behavioral health setting. Knowledge of computer skills to be able to search and enter data in the Electronic Record system is required.
Pay: From $46,000.00 per year
