What You'll Do

• Assess, monitor, and treat residents with wounds and skin integrity concerns.
• Develop and implement individualized wound care treatment plans.
• Perform wound assessments, measurements, and dressing changes.
• Collaborate with physicians, therapy, and nursing staff to promote optimal healing.
• Educate residents, families, and staff on wound prevention and treatment.
• Ensure accurate documentation and compliance with clinical standards and regulations.
• Lead initiatives focused on pressure injury prevention and skin health.
  
  

Qualifications

• Current, active Louisiana RN or LPN license.
• Wound care experience in a skilled nursing, rehabilitation, hospital, or long-term care setting preferred.
• Wound Care Certification (WCC/WOCN) is a plus but not required.
• Excellent assessment, communication, and organizational skills.
• A passion for delivering exceptional resident-centered care.
