The Children & Family Health Institute (CFHI) at VNA Health Group is seeking a dedicated Home Visitor RN to deliver essential care and support through the Nurse-Family Partnership (NFP) program in Somerset County, New Jersey. This role offers a unique opportunity to guide families toward healthier pregnancies, positive child development, and brighter futures.

As a Home Visitor RN, you will:
- Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients.
- Assess and address the health and developmental needs of women and their families.
- Offer counseling and instruction to help families achieve targeted health and life goals.
- Build and maintain strong community relationships to facilitate client referrals and resource connections.
- Attend weekly team meetings, case conferences, and clinical supervision sessions.
- Mentor and precept new hires and support student nurses during home visits as needed.
- Ensure compliance with agency infection control and safety policies.
- Maintain professionalism in adherence to organizational policies on absenteeism and appearance.
Qualifications
- Bachelor of Science in Nursing (BSN) required.
- 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred.
- Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities.
- Bilingual (English and Spanish-speaking) required.
- A commitment to CFHI's mission and the ability to build trusting relationships with clients and colleagues.
This position is covered by a collective bargaining agreement and is represented by a Union.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000.00 to USD $110, When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
