Position Summary:
The Family Advocate (FA) assists families in identifying and accessing resources. The FA supports families in need by connecting them with essential services. Through home visits and community engagement, the FA provides translation, referrals for furniture, clothing, food, healthcare, and educational resources. The FA also fosters ongoing relationships with clients, ensuring continued support and access to community programs. The program’s family-centered approach is grounded in key principles: treating families with respect and dignity, offering individualized and responsive support, and sharing information that empowers families to make informed decisions.
Principal Responsibilities:
Supports organizations’ mission by striving for excellence in all all aspects of their job with a focus on positive interpersonal relations with co-workers
Attends community outreach events and activities
Establishes a network of trust within the community’s residents in order to offer needed community case management and resources
Acquires and compiles intake information from residents
Adheres to the organization’s policy in regard to absenteeism and appearance
Job Competencies & Minimal Qualifications:
Associate’s/Bachelor’s degree preferred or a combination of education and experience
1-3 years of community health experience preferred; experience facilitating and conducting community education events is helpful
Bilingual; Spanish preferred
Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population
Excellent verbal and written communication skills
Strong organizational and interpersonal skills
Valid and current driver’s license
Working Conditions/Physical Demand: Various community settings including home visits, able to drive a car; use a laptop, cell phone other office equipment and programs

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $30,000.00 to USD $65,00.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
