Description
Job Summary: The Registered Nurse (RN) plans, organizes, and directs patient care with a focus on community health education and experience. The RN utilizes community resources to provide home and community-based care for individuals and families, ensuring their health needs are met.
Employment Status: Full-Time | Monday-Friday
Sign-On Bonus $5,000
Key Responsibilities:
- Conduct comprehensive assessments of patients and families to identify needs.
- Develop, implement, and revise individualized care plans.
- Administer medications and treatments as prescribed.
- Provide health education and counseling to patients and families.
- Act as a Case Manager, coordinating care and ensuring continuity.
- Maintain accurate clinical notes and communicate with physicians and health professionals.
- Participate in on-call duties and interdisciplinary team meetings
Qualifications
Qualifications:
- Graduate of an accredited nursing school with 1-2 years of acute care and home care experience.
- Active nursing license in Missouri and Kansas, with CPR certification.
- Valid driver’s license and insured automobile.
- Strong communication, problem-solving, and nursing skills.
