What are the responsibilities and job description for the RN Clinical Supervisor position at VNA Alliance?
VNA Alliance is a faith based, non-profit organization based in Meadville, Pennsylvania. VNA Alliance serves Crawford, Mercer, Venango, Erie, Forest, Warren, Butler, and Lawrence counties. We offer skilled nursing, palliative care, and comprehensive therapy services.
A. Reviews clinical documentation for compliance and completeness in a timely manner.
B. Reviews OASIS and Plan of Care (485) for accuracy and completeness.
C. Completes medication reconciliation, as appropriate
D. Reviews and approves the referral for accuracy and completeness in the electronic medical record. Ensures Intake Communication note is thorough and complete. Communicates with the Clinical Managers and/or scheduling to ensure the patient is scheduled appropriately for their level of acuity.
E. Communicates all patient hospitalizations and other pertinent information to the appropriate staff. Once a referral is received, follows patients until admission to the agency.
F. Directs members of the patient care team to assure that a high quality of care is provided.
G. Evaluation of patient’s condition for the need of other services, equipment, and consultations that may facilitate better overall outcomes.
H. Holds regular meetings to provide education to team members and facilitate interdisciplinary communication.
I. In collaboration with the primary clinician, provides what is necessary for the patient’s need to obtain positive medical outcomes while managing appropriate resources.
J. Plans with the primary clinician and other disciplines for the discharge from service at the time of admission.
K. May review and approve LPN verbal orders
L. Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
M. Liaison between patient/family, physician and all other healthcare providers.
N. Accepts and enters into the medical record workflow from authorized practitioners (e.g. IV orders for IV push medication, chemotherapy agents, etc…) that are outside a Licensed Practical Nurse (LPN) scope of practice
O. Provides in-home nursing visits on an as-needed basis.
P. Participates in administrative on-call rotation.
Q. Enters OASIS assessments into the electronic medical record, as required
R. Creates and/or approves Home Health Aide patient care plans
S. Responsible for understanding patient outcome and HHCAHPS requirements and assisting with education of the staff as it relates to these regulatory requirements
T. Reviews Telehealth data and identifies negative results. Collaborates with patient, clinical staff and physicians when negative results or trends are identified.
U. Other duties as assigned
DOCUMENTATION
A. Exhibits a thorough knowledge of the agency’s electronic health record documentation system.
B. Demonstrates expertise in all aspects of required documentation completion within scope of the employee’s licensure.
COMMUNICATION
A. Ensures the accuracy of public information materials and activities, representing the agency in professional settings and within the community-at-large.
B. Interacts with staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives as necessary.
C. Documents all communications in the clinical portion of the Telehealth monitoring system.

- Registered Nurse with a current, valid Pennsylvania license.
- OASIS Certified designation preferred
- Two [2] years of progressive experience in a home health care setting preferred.
- Interpersonal, communication and organizational skills.
- Proficient with knowledge of an electronic health record.
- Knowledge of agency policies, both federal and state regulations and third-party payers
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
