What You’ll Do
Provide on-site medical care to skilled nursing and long-term care patients, including management of acute and chronic conditions, post-hospital care, and coordination with nursing, therapy, specialists, and facility leadership.
Who We’re Looking For
- Oregon NP/PA license, or eligibility to obtain one
- National certification and DEA eligibility
- Strong clinical judgment and communication skills
- Efficient, independent, and highly motivated
SNF experience is helpful but not required. Exceptional new graduates are encouraged to apply and will receive structured onboarding and mentorship.
