What are the responsibilities and job description for the Youth Case Manager position at Vital Health?
Vital Health is a growing behavioral health and primary care center serving adults and children in Northwest Ohio. We provide trauma-informed, whole-person care and are looking for a Children's Case Manager to support youth in our day treatment program.
Schedule: School-day hours or 10am–8pm, Mon–Thurs.
What You'll Do:
- Provide case management for school-age youth
- Work with children in the clinic, community, and group settings
- Support behavior modification, life-skills, goal setting, and treatment planning
- Collaborate with families, schools, and community partners
- Complete timely documentation and maintain compliance
- Assist with transportation when needed
Requirements:
- 2 years case management experience
- 2 years behavioral health experience (youth preferred)
- QMHS experience preferred
- Active CDCA or CHW a plus
- Previous Ohio Medicaid provider number preferred
- Strong engagement, communication, organization, and computer skills
- Ability to work with culturally diverse youth and families
