Job Summary

Duties
- Develop and implement individualized case plans tailored to each youth’s unique needs, including educational, emotional, and social goals
- Coordinate services such as counseling, healthcare, educational support, and childcare to ensure comprehensive care
- Conduct regular home visits and assessments to monitor progress and address emerging issues promptly
- Collaborate with families, schools, foster care providers, and community organizations to build a strong support network for youth
- Advocate for children’s rights and safety by ensuring compliance with child protective services protocols and child welfare regulations
- Provide crisis intervention when necessary, including de-escalation techniques and emergency response coordination
- Maintain detailed case documentation, progress reports, and service records in accordance with agency policies
Experience
- Prior experience working with children or adolescents in social work, child welfare, or related fields is highly desirable
- Knowledge of child protective services procedures and familiarity with working with individuals with developmental disabilities enhances effectiveness in this role
- Experience in adoption processes or foster care systems is a plus
- Strong understanding of child welfare laws, regulations, and best practices for working with children and families
- Excellent communication skills to engage effectively with youth, families, and multidisciplinary teams
- Demonstrated ability to handle sensitive situations with empathy, professionalism, and transparency
