What You'll Do
As a Home Care Registered Nurse (RN), you'll provide skilled nursing care in patients' homes, including:
- Conducting comprehensive nursing assessments and developing individualized plans of care
- Completing OASIS Start of Care, Resumption of Care, Recertification, and Discharge assessments
- Wound care and dressing changes
- Medication management and patient education
- Catheter, ostomy, and other skilled nursing care
- Disease management and chronic care education
- Coordinating care with physicians and interdisciplinary healthcare teams
- Monitoring patient progress and identifying changes in condition
- Ordering medical supplies as needed
- Completing accurate, timely electronic documentation

Requirements
Need to have:
* Current Massachusetts Registered Nurse (RN) license
* Minimum of one (1) year of clinical nursing experience
* Afternoon availability
Nice to Have:
* Previous home care experience
* Monday–Friday availability
