What You'll Do
As a Home Care LPN, you'll provide skilled nursing care in patients' homes, including:
- Wound care
- Medication management
- Catheter care
- Diabetic care and education
- Wellness assessments
- Disease management education
- Follow-up after physician visits
- Ordering medical supplies as needed
- Accurate, timely electronic documentation

Requirements
Need to have:
* Current Massachusetts Licensed Practical Nurse (LPN) license
* Minimum of one (1) year of clinical nursing experience
* Afternoon availability
Nice to Have:
* Previous home care experience
* Monday–Friday availability
