Our Agency is seeking an EXPERIENCED REGISTERED NURSE - HOSPICE CASE MANAGER to join our progressive interdisciplinary team of clinicians, chaplain, social work, home health aides and volunteers. The RN Case Manager will have a caseload of palliative and hospice patients. The agency adheres to National Best Practice Guidelines for caseloads and visit schedules. We offer a flexible work environment and a competitive benefits package.

Weekend and holiday rotation. As a non-profit serving our community for over 100 years, our mission is to provide excellent, comprehensive and compassionate home care, hospice and preventative care to the residents of Litchfield County and the Farmington Valley.
