Hospice Eval LPN – Full Time (Home Hospice, Indiana)

Schedule: Monday–Friday On-Call Rotation

Location: Porter, LaPorte, Jasper, Starke, Newton, and Lake counties
About the Role
We are hiring a Hospice Evaluation LPN to join our home-based hospice team. This position is ideal for an LPN who is confident in the field, comfortable working independently, and skilled at supporting families during the first steps of hospice care.
You will serve as our rapid-response clinician, completing timely evaluations for new referrals and guiding families through consents and admission paperwork.
Responsibilities
- Respond promptly to new hospice referrals
- Conduct evaluations to determine hospice eligibility
- Educate patients and families on hospice services and care expectations
- Complete all required consents and admission documentation
- Coordinate with the interdisciplinary team to ensure smooth transitions
- Participate in the on-call rotation
Qualifications
- Active LPN license in the state of Indiana
- Strong assessment and communication skills
- Ability to work independently in the field
- Compassionate approach to patient and family support
- Reliable transportation and willingness to travel within the service area
