As a Physical Therapist, you will:
- Provide in-home physical therapy treatments under the direction of a licensed Physical Therapist and established plan of care.
- Carry out therapeutic exercises and functional activities while monitoring and reporting patient response and progress.
- Maintain accurate, timely documentation including visit notes, time, and mileage per agency guidelines.
- Communicate and collaborate with the Physical Therapist and interdisciplinary team to ensure coordinated patient care.
To qualify for a Physical Therapist position with us:
- Education: Associate’s degree from an accredited Physical Therapist Assistant program.
- Licensure: Must be licensed to practice as a Physical Therapy Assistant in the State of Indiana.
- Experience: At least two years of experience required; geriatric or home health experience preferred.
- Additional Requirements:
- Valid driver’s license and automobile insurance
- Successful completion of a criminal background check, post-offer physical exam, and drug screening
- Flexibility to travel as needed
