As a Physical Therapist, you will:
- Evaluate patients referred by a physician, focusing on diagnosis-specific assessments.
- Maintain accurate and timely documentation, including daily time and mileage records, evaluations, and progress notes, in accordance with agency policies.
- Schedule patients for treatment as needed.
- Collaborate with other healthcare providers and disciplines to ensure cohesive patient care.
- Communicate with the physician and VNA staff regarding patient’s treatment progress and status.
To qualify for a Physical Therapist position with us:
- Education: Bachelor’s degree in Physical Therapy from an accredited school.
- Licensure: Must be licensed to practice as a Physical Therapist in the State of Indiana.
- Experience: At least two years of experience required; geriatric or home health experience preferred.
- Additional Requirements:
- Valid driver’s license and automobile insurance
- Successful completion of a criminal background check, post-offer physical exam, and drug screening
- Flexibility to travel as needed
