NA Health Group is looking for a dedicated and organized Patient Service Coordinator to join our team In this key role, you will help support our mission by ensuring patients receive the care they need—on time and with compassion. You'll be responsible for scheduling visits, running reports in HCHB (Homecare Homebase), and maintaining communication with clinicians, patients, and Patient Care Directors.
Schedule and update patient visits based on care needs
- Run and maintain essential reports (e.g., productivity, case counts, missed visits)
- Answer incoming staff calls and refer clinical questions appropriately
- Coordinate with the Reimbursement team on private pay/managed care lists
- Uphold VNA Health Group's policies on attendance and professional appearance
- Perform other related duties as needed

Qualifications:
- High School Diploma or GED required (Associate's or Bachelor's degree preferred)
- 2–3 years of office experience; healthcare or medical office setting preferred
- Strong computer skills, including Microsoft Office
- Familiarity with HCHB is a plus
- Excellent organizational, interpersonal, and customer service skills.
Working Conditions/Physical Demand
: Business Office Environment with heavy phone and computer use

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $30,205.50 to USD $60, When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
