In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000.00 to USD $110,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits. What You’ll Do:
- Support Patients and Families: Conduct psychosocial assessments to identify patient needs and develop individualized care plans.
- Coordinate Care: Collaborate with nurses, therapists, physicians, and community resources to ensure seamless, holistic support.
- Empower Through Education: Provide counseling, crisis intervention, and education to patients and caregivers.
- Advocate for Access: Connect patients and families with community resources, financial aid, and emotional support services.
- Document with Purpose: Maintain accurate, timely documentation to ensure continuity and quality of care.
What We’re Looking for in You:
We’re searching for a dedicated professional who combines clinical expertise with compassion, empathy, and integrity.
- Current Licensed Social Worker (LSW) in the State of New Jersey (LCSW preferred).
- Master’s degree in Social Work from an accredited school.
- Minimum of 1 year of social work experience (homecare or healthcare experience preferred).
- Bilingual proficiency in Spanish and English required.
- Excellent communication and organizational skills.
- Proficiency with EMR systems and comfort with technology.
- Valid driver’s license and reliable vehicle – this is a field-based role.
- Flexibility to work both indoors and in the community.
- Frequent use of phone, computer, and clinical tools to document and communicate.
- Active engagement with patients, caregivers, and community partners – your presence makes all the difference.
