Responsibilities of Hospice Nurse Case Manager include:
- Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
- Supervise other home care staff as necessary.
Qualifications of a Hospice Nurse Case Manager include:
- Must possess an active RN license in both VT and NH.
- ASN required, BSN or MSN preferred.
- Nights, Weekend, Holiday and on-call rotation required.
