Responsibilities of a Nurse Case Manager include:
- Manage a case load of home care patients.
- Assess, plan, implement, coordinate, and evaluate care for each patient.
- Appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
- Supervise Home Care staff as necessary.
Qualifications for a Nurse Case Manager include:
- ASN required, BSN or MSN preferred.
- Must possess an active RN license in both VT and NH.
- Weekend and on-call rotation required.
